Procedure for Photocopy Requests:

UCHC faculty, staff, and students may use the form on this page to submit photocopy requests using FOAPAL coding or credit card.
UCHC patrons who wish to pay via cash or check may print out and submit this form.
Non-UCHC patron information is available here.

PHOTOCOPY REQUEST FORM FOR HEALTH CENTER FACULTY, STAFF AND STUDENTS

Library staff will access and send health-related journal articles from the UCHC Library collection QUICKLY and EFFICIENTLY on a prepaid basis. You may request OVERNIGHT service with photocopies available the next day after this form is submitted, or SAME DAY-RUSH service if the request is received by 2pm, Monday through Friday.
